THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU OR YOUR CHILD MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

SoCal Kids Therapy is committed to protecting the privacy of health information. This Notice of Privacy Practices describes how we may use and disclose protected health information (“PHI”) and explains the rights you and your child have regarding that information.

This notice should be finalized based on your actual HIPAA status, services, business structure, and information-sharing practices.

Your Rights

You have rights regarding your protected health information, including the right to:

  • Obtain a copy of your health information in paper or electronic form, subject to applicable law.

  • Request corrections to health information you believe is inaccurate or incomplete.

  • Request confidential communications, such as asking us to contact you in a particular way or at a particular location.

  • Request restrictions on certain uses or disclosures of your health information.

  • Request an accounting of certain disclosures of your health information.

  • Receive a copy of this Notice of Privacy Practices.

  • Choose a personal representative who may exercise certain rights on your behalf when permitted by law.

  • File a complaint if you believe your privacy rights have been violated.

For pediatric services, a parent or legal representative may generally exercise applicable rights on behalf of a child, subject to applicable law.

How We May Use and Disclose Your Information

We may use or disclose protected health information as permitted or required by law for purposes including:

Treatment

We may use or disclose information to provide, coordinate, or manage occupational therapy services.

For example, information may be shared with other healthcare professionals involved in your child's care when permitted by law.

Payment

We may use or disclose information as necessary to obtain payment for services.

Healthcare Operations

We may use information for activities necessary to operate our practice, including quality improvement, administrative activities, training, and business management.

HIPAA requires covered entities to describe permitted uses and disclosures, including treatment, payment, and healthcare operations, in their notice.

Other Permitted Uses and Disclosures

We may also use or disclose health information when permitted or required by applicable law, including circumstances involving:

  • Public health activities

  • Reporting certain required information

  • Health oversight activities

  • Judicial or administrative proceedings

  • Law enforcement requests when legally permitted

  • Serious threats to health or safety

  • Workers' compensation purposes when applicable

  • Other uses or disclosures required by law

Uses Requiring Your Authorization

Certain uses and disclosures of protected health information may require your written authorization.

If an authorization is required, we will obtain your written permission before using or disclosing information for that purpose, unless otherwise permitted or required by law.

You may generally revoke an authorization in writing, subject to actions already taken based on the authorization.

Our Responsibilities

SoCal Kids Therapy is required by law, when HIPAA applies, to:

  • Maintain the privacy of protected health information

  • Provide you with this Notice of Privacy Practices

  • Follow the terms of the notice currently in effect

  • Notify affected individuals as required by law if a breach compromises the privacy or security of protected health information

  • Provide you with information about your privacy rights

HHS identifies these as core responsibilities of covered healthcare providers.

Changes to This Notice

We may change this Notice of Privacy Practices as our privacy practices or applicable laws change.

The updated notice will be available on our website and upon request.
